About the Role
We are seeking an experienced and dedicated Care Manager to lead and support our clinical team. In this full-time position, you will oversee the coordination, delivery, and evaluation of high-quality care services aligned with the Aged Care Quality Standards. You will mentor staff, support residents and families, and contribute to a positive, person-centred care culture.
Key Responsibilities

Oversee clinical care delivery and ensure compliance with Aged Care Quality Standards

Lead, mentor, and support the nursing and care teams

Conduct clinical assessments, care planning, and care reviews

Collaborate with residents, families, and allied health professionals

Support accreditation activities and continuous improvement initiatives

Maintain accurate documentation and reporting
About You
- Minimum 5 years’ experience as a Clinical Nurse, Care Coordinator, or Care Manager
- At least 2 years’ experience in aged care

Strong working knowledge of the Aged Care Quality Standards

Excellent leadership, communication, and problem-solving skills

A commitment to delivering high-quality, person-centred care

Current AHPRA registration (RN)
