As CCM, you will play a pivotal role in overseeing clinical governance, ensuring compliance, and leading a dedicated care team to deliver high-quality, person-centred care to our residents.
Key Responsibilities

Oversee clinical care delivery and ensure compliance with Aged Care Quality Standards

Lead, mentor, and support the nursing and care teams

Conduct clinical assessments, care planning, and care reviews
-
Collaborate with residents, families, and allied health professionals
-
Support accreditation activities and continuous improvement initiatives
-
Maintain accurate documentation and reporting
-
Minimum 5 years’ experience as a Clinical Nurse, Care Coordinator, or Care Manager

At least 2 years’ experience in aged care
-
Strong working knowledge of the Aged Care Quality Standards

Excellent leadership, communication, and problem-solving skills

A commitment to delivering high-quality, person-centred care

Current AHPRA registration (RN)
